In order to cover a preexisting condition(s), you
may need to provide your new insurer with a Certificate of Creditable Coverage.
| If you expect to
purchase your own health insurance or receive health insurance through an
employer within the next two months, in order to avoid being denied payment
for services for preexisting health conditions, you may need to provide your
health insurer with a Certificate of Creditable Coverage, which will provide
proof of continuous medical coverage. |
To request this Certificate of Creditable Coverage, call
(609) 588-3721 (24 hr. / day) and be prepared to leave the following information
at the voice prompt:
- Full Name
- Social Security Number
- 12 digit Medicaid Number
Allow up to 3 weeks for delivery of your certificate.
