How to Apply for NJ FamilyCare
It's easy. Here's how to
apply.
- To get an application go to the
Application and Downloads page on this Web site or call 201-368-4200
and ask for a NJ FamilyCare application. Give your name and
address and an application package will be mailed to you.
- You should file your application in
the county where you live. Failing to do so could result in a
delay in processing your application. If you are not sure if
you are a Bergen County resident, click here to see list of
towns and counties in the North Jersey Area.
- As soon as you receive your
application packet complete the Application and HMO Selection Form.
- Check the list of required
verifications in the application. Make copies of all of the
required documents. Make sure you keep the originals, return the
copies with your application.
- Check the HMO
Comparison Web page to obtain more information about the NJ
FamilyCare HMOs and to help you with your HMO selection.
- Sign the Application and the HMO
Selection Form.
- Place everything in the Postage Paid
Return Envelope and drop it in the mail.
